Part time hours consideredDynamic role where no 2 days are the same!Our Carer Gateway is an initiative to provide carers (anyone who cares for a friend or family member who is frail, ill, has disability or is an older Australian) with information and support to help them continue their fundamental role happily and healthily. Our service offers short term emergency help including Respite, Counselling and Coaching. As our Support Coordinator, you will Assess and investigate the needs of the carer to point them in the right direction. Work collaboratively in a fast paced environment with great negotiation skillsDevelop initial service plans in collaboration with the carer, medical and allied health professionals and referring agencies as appropriate, ensuring it meets the carers needs, goals, preferences and eligibility.Think outside the box and will use your skills and knowledge to navigate your way seamlessly through our funding systems.   our service is predominantly a phone based service where you will connect and build make a positive impact in our carers lives that will last a lifetime!  What we need from you? Experience as a Case Manager, Support Coordinator or Social Worker A passion for helping our clients live their best lives while they support a loved one to have a better quality of life.
