The program aims to reduce risks and strengthen families to reduce the number of children entering Out of Home Care.About these roles:Partnering with a small number of at risk families, you will undertake an initial assessment of the family’s strengths and needs. Your case plans will empower families to make lasting change, build upon their resilience to reduce the risk of significant harm and make referrals to appropriate networks.As our Case Worker/Social Worker, you will work as part of a specialist multidisciplinary team based from our new offices in Tamworth.Terms: This is a community based role in Cowra the closest hub to work from will be Orange/Parks.
