Contract role until 31 December 2024Based primarily in Capalaba, however some travel between Capalaba and Loganholme Hubs will be required$84k - $99k inclusive of Super + Salary PackagingFlexible Friendly organisation, hybrid work options available!About the roleFamily and Child Connect (FaCC) services have been established across Queensland to support families who are at risk of entering or re-entering the child protection system.FaCC are local, community-based services that help families to care for and protect their children at home, by connecting them to the right services at the right time. Broadly, the FaCC will:Assess the information they receive with a focus on the safety and protection of the children in the familyProvide advice where appropriateMake contact with the family to discuss the concernsAssess the type and level of support requiredRefer the family to services to assist them, including Intensive Family Support services, andSupport a Local Level Alliance (LLA) of government and non-government services within the community to ensure that vulnerable families receive the right mix of services.As a Child & Family Practitioner with FaCC, you will undertake an initial assessment of family strengths and needs, determine an appropriate referral pathway for the family to help them achieve their goals, and follow up with services to ensure successful engagement. Typically we engage with families on a short term basis (6 weeks).DutiesConduct initial assessments of a family’s strengths and needs identifying their main goalsIdentify and make appropriate referrals based on the goals; and follow up with servicesProvide information and advice to families based on their individual circumstancesAssess and manage child protection risks for familiesLiaise with key stakeholders in the communityKeep accurate and timely documentation of all client interactions and related tasksOur Benefits   We support people to live their best life.
