Child & Family Practitioner (Identified)

Our Early Years Centres are community hubs offering child health services, family support and early childhood education whilst focusing on early intervention and prevention to ultimately support children and families to thrive.Your new role:Partnering with families, you will undertake an initial assessment of the family’s strengths and needs. Your case plans will empower families to make lasting change, build upon their resilience to respond to life’s challenges and make referrals to appropriate networks. You will also be co-facilitating playgroups and parenting programs.You will work across our Early Years Centre’s at Browns Plains, Acacia Ridge and Beaudesert. You will join a multi-disciplinary team including, Child Development Specialists, Child and Family Practitioners, Administrative staff, Early Childhood Educators and Teachers.  You will work alongside some of our partner services such as Child Health Queensland and Metro South Midwifery Group Practice. Our services also include a long day care and kindergarten to ensure full wraparound support and care for children and families.As part of your role, you will be required to build the cultural capability of the service and staff, ensuring appropriate delivery of services to Aboriginal and Torres Strait Islander families.
